Remplissage of Subscapularis Tendon

A 20-year-old patient presented for repair of right shoulder posterior instability, a Bankart lesion and reverse Hill-Sachs lesion. The arthroscope was introduced and examination revealed an injury to the posterior capsulolabral structures consistent with a bony Bankart fracture; this was repaired. Following repair of the posterior labrum, the subscapularis and the reverse Hill-Sachs lesion were visualized. The rotator interval was opened using a shaver, and with visualization of the subscapularis tendon, two Arthrex BioComposite SutureTak® suture anchors were then positioned into the reverse Hill-Sachs lesion through the inferior portal. The associated sutures were then passed through the subscapularis tendon and tied pulling the subscapularis tendon over to fill in the reverse Hill-Sachs lesion creating a remplissage effect, completing the modified McLaughlin procedure.
