Repair of Abdominal Aortic Aneurysm

When the terms “aneurysmectomy” and “dissection” of aneurysm, are documented should these procedures be coded as “excisions?” For example, a patient was admitted with an infrarenal abdominal aortic aneurysm. After extensive lysis of adhesions from a previous colectomy, the neck of the aneurysm was dissected from the surrounding structures in the infrarenal position. A bifurcated graft was trimmed to the appropriate size and sewn end-to-end to both iliac artery orifices. The aneurysmal sac was reapproximated over the graft using sutures. Our surgeons say that the blood vessels of the aneurysm are not “resected” and that the procedures listed in subcategory 38.4X, Resection of vessel with replacement, are obsolete. Some pathology reports even indicate the removal of atherosclerotic plaque and thrombus and not sections of blood vessel walls with aneurysmal lumen. ...

Article Overview

This premium coding article addresses documentation language used in abdominal aortic aneurysm repair and the coding implications of operative terms such as aneurysmectomy, dissection, and vessel replacement. It is intended for coders, CDI staff, and billing professionals working with vascular surgery records, operative reports, and procedure classification guidance.

Why This Topic Matters

Abdominal aortic aneurysm repair cases often contain operative language that can be difficult to map consistently to procedure coding systems. This article helps readers understand the coding context for vessel repair documentation in vascular surgery records.

What You Will Learn

  • How abdominal aortic aneurysm repair terminology is discussed in relation to coding classification
  • What documentation concepts are relevant when reviewing operative reports for vascular procedures
  • Which coding frameworks are implicated by vessel repair and replacement language
  • How coders can approach terminology differences in surgical documentation for vascular cases

Who Should Read This
