Resection of Sellar Craniopharyngioma
A patient was admitted for endoscopic transsphenoidal resection of a large sellar mass with suprasellar extension that was suspicious for craniopharyngioma. Bone was removed up to the anterior portion where the tumor could be identified on neuronavigation. Along the right inferior aspect of the lesion, a large mucus-containing cyst was identified. This was fully evacuated and further opened to identify a yellowish granulated tumor with significant calcifications and adherence to the floor of the sella. The tumor was removed using a combination of ring curettes and biting instruments and was separated carefully from the arachnoid capsule. Next, a portion of the tumor along the left side, noted to be adherent to the hypophyseal vessel, was removed using pituitary rongeurs. The tumor was also removed along the floor of the sella.
