AHA Coding Clinic® for ICD-9 - 2007 Issue 4; Volume 3 New/Revised Codes
Spinal Motion Preservation Procedures
Effective October 1, 2007, a new subcategory (84.8) has been created to report the insertion, replacement and revision of posterior spinal motion preservation devices. The new procedure codes have been created to distinguish different types of motion preservation technologies being used. These devices are also called dynamic spinal stabilization devices. Prior to the creation of the new codes, several of the procedures were captured with code 84.59, Insertion of other spinal devices. Subcategory 84.8 includes any synchronous facetectomy (partial or total) performed at the same level. However, if a synchronous surgical decompression (foraminotomy, laminectomy, or laminotomy)...

Article Overview
This article covers the introduction of an ICD-9-CM procedure coding subcategory for posterior spinal motion preservation procedures and related device types. It is useful for coders, CDI staff, reimbursement teams, and orthopedic or spine practices that need to understand the scope of the topic, the affected ICD-9-CM procedure code groupings, related exclusions, and the diagnostic context discussed in the article. The article also points readers to related coding guidance and a sample coding discussion involving spine conditions and decompression.
Why This Topic Matters
Spine procedures can involve overlapping device placement, revision, decompression, and fusion-related services, so understanding the article’s scope helps readers identify when the premium guidance is relevant. It also helps coding professionals see how the topic was framed around specific ICD-9-CM procedure categories and related diagnosis context.
Article Sections
-
Overview of the new posterior spinal motion preservation subcategory
Introduces the new procedure grouping and the general purpose of distinguishing posterior spinal motion preservation technologies. It also places the topic in the context of earlier coding practices and related spine procedures.
-
Subcategory scope and associated decompression guidance
Summarizes the kinds of posterior spinal procedures included in the subcategory and discusses when related decompression services are considered in the article. The section also frames the relationship to broader spine stabilization concepts.
-
Motion preservation technology categories
Outlines the main categories of posterior column motion preservation technologies discussed in the article. It provides a broad classification framework without detailing coding choices.
-
Interspinous process devices
Covers one device category, including its clinical context, examples, and the associated coding update discussed in the article. The section also notes the related revision topic.
-
Pedicle-based dynamic stabilization devices
Describes another posterior stabilization category, its intended clinical use, examples, and the related insertion and revision coding topics. It also distinguishes this category from other spine hardware references.
-
Facet replacement devices
Reviews the facet replacement category, its clinical context, examples, and the related insertion and revision coding topics. This section is focused on the device class and its place within the broader motion preservation discussion.
-
Intervertebral disc replacements and disc repair systems
Addresses additional motion preservation technologies and how the article situates them relative to existing spinal disc replacement coding categories. It also references an external coding resource for further information.
-
Clinical example and coding discussion
Presents a sample patient scenario involving spinal conditions and a posterior spinal device procedure, followed by a coding-oriented discussion. The section is meant to illustrate the article’s application context without reproducing the full guidance.
