AHA Coding Clinic® for ICD-9 - 2012 Issue 1
Total Laparoscopic Abdominal Hysterectomy and Laparoscopically Assisted Vaginal Hysterectomy
The Central Office on ICD-9-CM has recently received questions related to the differences in code assignment between total laparoscopic abdominal hysterectomy (TLH) procedures and laparoscopically assisted vaginal hysterectomy procedures (LAVH). Effective October 1, 2006, new procedure codes were created to distinguish between laparoscopic hysterectomies that utilize tiny incisions and traditional hysterectomies that require a large incision. For additional information on the other types of laparoscopic hysterectomy procedures, please refer to Coding Clinic, Fourth Quarter 2006, pages 130-134. An important factor in assigning the correct ICD-9-CM hysterectomy procedure code is to determine what structures were detached and how they were...

Article Overview
This Coding Clinic article addresses how ICD-9-CM procedure coding differentiates laparoscopic hysterectomy approaches based on the operative technique and structures detached, rather than the final route of specimen removal. It is relevant for inpatient coders, HIM professionals, and clinicians documenting hysterectomy procedures, especially when laparoscopic and vaginal components are both present. The article also touches on how related gynecologic procedures and documentation details affect code assignment.
Why This Topic Matters
Accurate classification of hysterectomy approach affects ICD-9-CM procedure reporting and prevents confusion when the uterus is removed through the vagina after laparoscopic detachment.
