Transfusion Related Lung Injury (TRALI)
Effective October 1, 2006, code 518.7, has been created to identify transfusion related acute lung injury (TRALI). TRALI is a reaction to the transfusion of whole blood, packed red blood cells, fresh frozen plasma and platelets. Symptoms of dyspnea, cyanosis, hypotension, fever, and bilateral pulmonary edema with normal cardiac function, usually develop within 1-2 hours and are present 4-6 hours after transfusion. While the severity of TRALI ranges from mild to severe, the condition has been reported as the third most common cause of fatal reactions to transfusion. Treatment is based on oxygen therapy and hemodynamic support. When...

Article Overview
This article covers a coding update tied to transfusion-related acute lung injury (TRALI), including the effective date, the clinical setting in which the condition may arise, and a brief coding example. It is relevant to coders, CDI teams, compliance staff, and clinicians who work with blood product transfusions and adverse event documentation. The article provides general background on the condition, its presentation, and how the update fits into diagnosis coding, without serving as a substitute for the full premium guidance.
Why This Topic Matters
TRALI is an important transfusion-associated complication that can affect diagnosis coding, patient record accuracy, and adverse event reporting. Understanding the coding update helps users recognize when documentation may support the newly introduced diagnosis category discussed in the article.
