AHA Coding Clinic® for ICD-9 - 2011 Issue 4; Volume 3 New/Revised Codes
Ventricular Drainage
Effective October 1, 2011, code 02.2, Ventriculostomy, has been expanded in order to clarify the appropriate code assignment for procedures involving ventricular drainage using a catheter or shunt. Prior to this change, codes 02.2, Ventriculostomy, and 02.39, Other operations to establish drainage of ventricle, were used to describe these procedures. The changes will help clarify that ventricular drains are external and are classified to 02.21, Insertion or replacement of external ventricular drain [EVD]; while ventricular shunts are intracranial and should be assigned to 02.22, Intracranial ventricular shunt or anastomosis. In addition, the title of...

Article Overview
This article reviews a coding update for ventricular drainage procedures, with emphasis on how related intracranial and extracranial drainage procedures are organized in ICD-10-PCS. It is intended for coding professionals, CDI specialists, and others who need to understand the scope of the revision, the affected code set structure, and the general clinical contexts in which these procedures are performed.
Why This Topic Matters
Accurate classification of ventricular drainage procedures affects code assignment for neurologic care, procedural documentation review, and reporting consistency. The article is relevant to anyone working with trauma, neurosurgery, hydrocephalus, or intracranial pressure management documentation.
Article Sections
-
Coding update overview
Introduces the coding change and the procedural topics it affects. Summarizes the broader distinction being addressed within the procedure classification.
-
Clinical context for ventricular drainage
Provides background on the clinical situations in which ventricular drainage may be used. Discusses general monitoring and drainage purposes in neurologic care.
-
ICD-10-PCS code structure changes
Describes the affected subcategory and revised code organization. Includes the procedural groupings and related exclusions that were updated.
-
Documentation example and coding question
Presents a short case scenario and asks how the procedure should be coded. Follows with a coding-oriented response tied to the documented procedure.
-
Operative report example
Introduces another documentation example drawn from an operative report. The text continues with procedural language relevant to coding review.
