The AMA and HCFA are committed to continually improving the Documentation Guidelines and welcome recommended revisions based on their usage. Any recommendations at this time can be submitted to Celeste Kirschner, at the AMA or to Bart McCann, MD, at HCFA. Subsequent to CPT 1999, requests for revisions will be submitted according to the CPT process.
