Hysterosonography and Hysterosalpingography (July 1999)

July 1999 pages 8-9 Coding Communication Hysterosonography and Hysterosalpingography The most common use of hysterosonography is for evaluation of patients with dysfunctional or postmenopausal bleeding when endometrial abnormalities (eg polyps, hyperplasia, carcinoma) are suspected. Hysterosonography may also be used to evaluate fallopian tube patency and endometrial cavity anatomy for infertility patients. Hysterosonography entered into widespread use over the past four years. Article Overview

This article reviews CPT guidance for hysterosonography and hysterosalpingography in the context of gynecologic imaging and infertility evaluation. It is relevant to coders, billers, and imaging practices that need to distinguish the separate components of these services, understand which CPT codes are associated with each part of the study, and recognize when modifiers may apply. The article also places the topic in historical context by contrasting earlier reporting approaches with the CPT codes referenced in the guidance.

Why This Topic Matters

Correct reporting of these services depends on separating the catheterization/injection component from the radiological supervision and interpretation component and understanding how related ultrasound services fit into the encounter. The article helps reduce inconsistent billing for common gynecologic diagnostic studies.

Article Sections

  1. Coding Communication

Introduces the reporting topic and explains the clinical context for hysterosonography and hysterosalpingography. It summarizes why the procedures became widely used and how they relate to gynecologic evaluation.

  2. A Look at the Codes

Reviews the coding background and the CPT references associated with these procedures. It contrasts earlier reporting approaches with the codes discussed in the article.

  3. What's Included?

Describes the general components addressed by the guidance and how the procedure pieces are separated in reporting. It also discusses the relationship between the imaging portion and related ultrasound services.

  4. How to Report the Services Provided

Explains the overall reporting framework based on who performs the components of the service. It also addresses professional-component reporting considerations.

  5. Hysterosonography Clinical Vignette

Presents illustrative patient scenarios and a procedural narrative showing common clinical settings for the study. The vignette supports understanding of the service context without substituting for the coding guidance.
