Using a sheath advanced into the right atrium, the PFO is measured with a sizing balloon, which is exchanged for an irrigated ablation catheter. To close the PFO, RF energy is delivered along the upper edge of the septum, guided by intracardiac echocardiography (ICE). After several minutes the catheters and sheaths are removed and hemostasis is achieved. What are the appropriate code(s) to report?
