December 2017 page 12 Nerve Repair with Nerve Allograft (64912, 64913) Prior to 2018, Current Procedural Terminology (CPT®) codes were available to report four types of grafts used for nerve repair: nerve autograft (64885-64907); vein allograft (64910); vein autograft (64911); and synthetic conduit (64910). For CPT 2018, two new codes (64912, 64913) have been added to report nerve repairs using nerve allografts. Previously, nerve repairs with nerve allografts were reported with unlisted code 64999, Unlisted procedure, nervous system.
