New CMS 1500 Claim Form Effective January 1, 2007 A new version of the CMS 1500 Health Insurance Claim Form (version 08/05) will be ready for use on January 1, 2007. The new form, however, will not be mandated for use until April 1, 2007. Until that time, providers may submit either the current version of the CMS 1500 claim form (12/90) or the new version (08/05).
