Would it be appropriate to report code 43195 or 43499 for a rigid esophagoscopy with Botox injection into the cricopharyngeus muscle? The hypopharynx was visualized with a rigid esophagoscope. A prominent cricopharyngeal bar was noted. A Weerda bivalve scope was then used to lift the larynx and visualize the post-cricoid area. The cricopharyngeus (CP) muscle was exposed using a 0-degree telescope for direct visualization. A red Rob-NelTM catheter was inserted into the esophageal lumen to confirm proper placement, which was achieved without difficulty. After removing the catheter, an esophageal balloon dilator was inserted and inflated to 18 mm; inflation was maintained for 120 seconds before deflating the balloon. Upon reexamination, there was no evidence of perforation or significant mucosal injury. Then the CP was re-exposed and a total of 30 units of Botox were injected into two separate locations.
