A lateral malleolus fracture nonunion is repaired via an open approach with internal fixation. Would it be appropriate to report CPT code 27726, Repair of fibula nonunion and/or malunion with internal fixation, even though the code does not read malleolus? Or should CPT code 27792, Open treatment of distal fibular fracture (lateral malleolus), includes internal fixation, when performed, be used instead with modifier 22 to reflect the additional work required to repair the nonunion?
