Are nerve graft codes reported based on each nerve or each extremity? For example, when two separate digital nerves in the third digit of the same hand are each repaired with a single-strand 3-cm graft, should two units of code 64890 be reported? Or should the length of the two grafts be totaled (ie, 3 cm + 3 cm = 6 cm) and code 64891 be reported?
