When our physicians report biopsies of the ear, eyelid and lip that involve skin and subcutaneous tissues only, we use CPT 11100. Is this correct? Under what circumstances would it be appropriate to use the site-specific biopsy codes 40490, 67810 (REVISED IN 2013) and 69100?
