Our practitioners are confused by the examination component for preventive services codes (99381-99387, 99391-99397)? Can you tell us what constitutes an "age and gender appropriate examination" for these services? Our practitioners are Internal Medicine and Family Practice physicians whose claims for these encounters by are being denied by our Auditing Department prior to posting because their documentation does not contain the equivalent documentation of the comprehensive 1997 physical examination per the CMS Documentation Guidelines, such as for an annual well woman examination or male examination.To view the Official AMA answer and 1000s more like this:
