When I grew up, my parents never had any health insurance.  We were lucky, with 7 kids, we rarely had to see a doctor.  Minor illnesses and injuries were treated with common sense, ointment, ice, heat, chicken soup and bed rest and we never ran to the emergency room when something happened.  In the 1950s and 1960s before you left the hospital, you were taken to the cashier where you didn't yell and scream about the bill.
