An Advance Beneficiary Notice (ABN) is a written communication given to a patient prior to providing a service that informs the patient that Medicare may not or will not cover the service. It is required when the service is sometimes, but not always, covered. If the service is never covered (cosmetic, for example) it is not required.
CMS revised its ABN form, and the use of the revised form is mandatory Nov 1 2011. The new form is attached to this article.
