New coding guidance: Physical therapy evaluations will require detailed documentation

Practices that provide physical therapy should take note of pending changes to evaluation coding. According to the proposed physician fee schedule (APCPS 8/16), CPT intends to delete the two existing evaluation codes and replace them with four new codes. Take steps now to make sure clinical and coding staff understand the work and documentation needed to meet the new codes’ detailed requirements.
