Q&A: Global period may apply when patient returns a week after fracture surgery

Question: We had an adult patient with a Monteggia fracture undergo surgery, which we billed with CPT 24635. She came back a week after surgery with subluxation of the radial head. They attempted closed reduction of the radial head and were able to improve alignment and place a cast. The only CPT code for reduction of radial head is 24640 (Closed treatment of radial head subluxation in child, nursemaid elbow, with manipulation) but that code states “child.” Not sure what code to use for an adult? Also can modifier 78 even be used with place of service 11? Any advice will be appreciated. Thank you.
