Question: How will the CMS 1500 form change to allow ICD-10-CM reporting? Vermont Subscriber Answer: Those providers exempt from electronic submission requirements and who use the paper CMS-1500 will have a new form available to use in 2014. The new version (02/12) provides space for 12 codes (instead of just 4) and allows providers to identify whether they’re using ICD-9-CM or ICD-10-CM codes.
