The Centers for Medicare & Medicaid Services (CMS) wants to improve access to quality care while encouraging coordination between providers. But sometimes that’s hard to do with incomplete patient histories and data. A new pilot program aims to combine the might of health IT and claims data, ensuring beneficiaries receive the best care possible by eradicating the inconsistencies.
