In the Third Quarter issue of AHA’s Coding Clinic for HCPCS, advice was provided to report CPT code 94667, Manipulation chest wall, such as cupping, percussing, and vibration to facilitate lung function; initial demonstration and/or evaluation, for the reporting of certain devices utilized to clear mucous secretions in the lungs.
In Second Quarter, 2011, the advice was changed to instruct the coder to report CPT code 94664, Demonstration and/or evaluation of patient utilization of an aerosol generator, nebulizer, metered dose inhaler or IPPB device, for the reporting of the device when utilized to clear mucous secretions in the lungs.
However, in the September 2010, issue of the AMA’s CPT Assistant, instruction was provided to the coder to report CPT codes 94667 and 94668, when performing chest wall manipulations for the clearing of the airway to improve lung function. We feel this issue may need clarification.
Based on extensive discussions through the American Medical Association (AMA) CPT review process it was confirmed that the Flutter and Acapella technique for clearing of mucous secretions in the lung would not be appropriately reported with CPT code 94667, Manipulation chest wall, such as cuppin, percussing, and vibration to facilitate lung function; initial demonstration an/or evaluation. The Flutter and Acapella technique is inhaled and does not involve chest wall manipulation.
