Hero Graft
Our facility is beginning to use the Hemodialysis Reliable Outflow (HeRO) Graft subcutaneous AV access device. It was recommended that both CPT codes 36830, Creation of arteriovenous fistula by other than direct arteriovenous anastomosis (separate procedure); nonautogenous graft (eg, biological collagen, thermoplastic graft), and CPT code 36558, Insertion of tunneled centrally inserted central venous catheter, without subcutaneous port or pump; age 5 years or older, should be reported for the implantation procedure. CPT codes 76937 or 77001 would be additionally reported if supported by the documentation.
However, since CPT code 36830 is considered a “separate procedure,” would both codes be used in the case of implanting a HeRO Graft?
