A patient with status post heart transplantation is seen at our facility for a right heart catheterization and endomyocardial biopsy with ultrasound guidance to evaluate allograft rejection and hemodynamics as a sign of rejection. What are the appropriate codes for the procedures performed?
Under the OPPS, report CPT code 93505, Endomyocardial biopsy, for the right heart catheterization and endomyocardial biopsy procedures performed. The right heart catheterization is utilized as an access to the right heart and considered a part of the heart biopsy and not separately reported. The ultrasound guidance for vascular access is not included in CPT code 93505 for the catheterization. Therefore, it would be appropriate to report CPT code 76937, Ultrasound guidance for vascular access requiring ultrasound evaluation of potential access sites, documentation of selected vessel patency, concurrent realtime ultrasound visualization of vascular needle entry, with permanent recording and reporting, for the ultrasound guidance performed. Please note, that if the right heart catheterization is performed for another diagnostic purpose then both codes would be appropriately reported.
