As a reminder, to hospitals and other providers, remember to append modifier JW, Drug amount discarded/not administered to any patient, to identify unused drug or biologicals from single use vials or single use packages that are appropriately discarded.
Hospitals and providers are reminded that payment is provided for the amount of drug or biological discarded as well as the dose administered, up to the amount of the drug or biological as indicated on the vial or package label. For example: a single use vial that is labeled to contain 100 units of a drug has 95 units administered to the patient and 5 units discarded. The 95 unit dose is billed on one line, while the discarded 5 units may be billed on another line with the listing of the JW modifier.
To ensure the appropriate reporting of the HCPCS code for the injection provided be guided by the dosage description listed in the HCPCS code descriptor to determine the correct units to be billed for the drug administered.
For additional information regarding the reporting of discarded drugs and biologicals refer to Chapter 17, Section 40, at http://www.cms.gov/Regulations-and-Guidance/Guidance/Manuals/Downloads/clm104c17.pdf.
