In response to the many questions received, the following current coding advice is given on cardiac pacemaker pocket procedures: Creation of a new pocket to replace the old pocket, 86.09. Cleaning (toiletry) of pocket involving debridement of tissue, 86.22. Refashioning or enlargement of existing pocket, 86.99. When the current site of the pacemaker unit interferes with the approach or site of a scheduled surgical procedure and necessitates repositioning the pacemaker and pocket to the opposite side, 37.89 + 86.09.
