Chronic Kidney Disease in a Patient with Hypertension Associated with Diabetes
A patient presented with type 2 diabetes mellitus with hyperglycemia. The provider documented that the diabetes is associated with hypertension. The patient also has stage 3b chronic kidney disease (CKD) that has not been linked to another condition. The ICD-10-CM classification presumes a relationship between hypertension and CKD unless the provider links the CKD to another condition. However, when referencing the ICD-10-CM Tabular List, at category I12, Hypertensive chronic kidney disease, there is an Excludes1 note that leads to category I15, Secondary hypertension, for hypertension that is secondary to another condition. Since codes in category I12 cannot be assigned with codes in category I15, what are the appropriate code assignments for hypertension due to diabetes mellitus in a patient with stage 3b CKD? ...
