There seems to be an extra sentence under the Activity Code Guidelines published in Coding Clinic that does not show up in the guidelines on the website (page 90): "Do not assign E030, Unspecified activity, if the activity is not stated." Please clarify whether an activity E code should be assigned when an external cause E code is reported, even if it is the unspecified activity? ...
