Pressure Ulcer Stages

Effective October 1, 2008, a new subcategory (707.2) has been created to report pressure ulcer stages. Prior to this change, the ICD-9-CM classified pressure ulcers by site, but did not distinguish the stages. The new codes (707.20-707.25) do not specify the site of the ulcer. Article Overview

This article covers the ICD-9-CM changes that introduced pressure ulcer staging, the relationship between ulcer site and stage coding, and the documentation sources used to support stage assignment. It is relevant for coders, CDI staff, and clinical documentation stakeholders who work with pressure ulcer diagnoses and the associated Official Guidelines for Coding and Reporting.

Why This Topic Matters

Accurate pressure ulcer coding depends on understanding the staging framework, how the stage codes fit with site reporting, and what documentation can support the final code assignment. The article also points readers to official guidance and clarification from the National Pressure Ulcer Advisory Panel.

Article Sections

  1. ICD-9-CM pressure ulcer staging update

Introduces the coding update that created a new staging subcategory and explains how it relates to existing pressure ulcer site reporting. It also notes the timing of the change and the broader classification context.

  2. NPUAP definition and staging context

Summarizes the pressure ulcer definition referenced by the National Pressure Ulcer Advisory Panel and its role in the staging framework. This section provides clinical context for the coding discussion.

  3. Index and documentation considerations

Discusses how the Alphabetic Index and provider documentation relate to pressure ulcer staging assignment. It also references guidance about documentation from clinicians other than the provider.

  4. ICD-9-CM code revisions for chronic ulcer and pressure ulcer staging

Presents the affected ICD-9-CM code areas and the revised staging subcategory structure. It includes the staged pressure ulcer code set and related revision notes.
