4A133B3 Monitoring of arterial pressure, pulmonary, percutaneous approach
A right cardiac catheterization code is not assigned along with the code for insertion of a Swan Ganz catheter unless there is documentation of a diagnostic cardiac catheterization and a report of the diagnostic catheterization in the health record.
