She was last seen the previous afternoon and was asymptomatic. Initial Telestroke was 9 on the National Institutes of Health (NIH) stroke scale; this improved to 4 (persistent dysarthria) upon arrival at the treating hospital. No tissue plasminogen activator was given. Computed tomographic angiography and magnetic resonance imaging demonstrated mid-basilar and left posterior cerebral artery (PCA) P1 occlusions along with brainstem infarcts.
