Documentation Challenge The Physician Quality Reporting Initiative (PQRI) is a Medicare claims-based quality reporting program enacted with the passage of the Tax Relief and Health Care Act of 2006. This voluntary reporting program was implemented by the Centers for Medicare and Medicaid Services (CMS) in July 2007.
