Intensity Modulated Radiation Therapy (IMRT): CPT Codes 77301, 77418, 77338 (November 2009)
November 2009 pages 3-5 Intensity Modulated Radiation Therapy (IMRT): CPT Codes 77301, 77418, 77338 Intensity Modulated Radiation Therapy (IMRT) is a technology for delivering highly conformal external beam radiation to solid tumors. When the treatment volume "conforms" to the shape of the tumor, the relative toxicity of radiation to the surrounding normal tissues is reduced, allowing a higher dose of radiation to be delivered to the tumor than conventional techniques would allow. IMRT results in more conformal target coverage and normal tissue sparing than three-dimensional (3-D) conformal radiotherapy. IMRT is particularly useful for obtaining the highly conformal dose distributions...

Article Overview
This article explains the IMRT process of care from planning through delivery and discusses how the associated CPT framework was organized at the time of publication. It is aimed at radiation oncology coders, billers, physicians, physicists, and other staff who document or review IMRT services. The article also covers general medical necessity themes, common clinical scenarios, and related guidance on planning methods, device construction, and treatment delivery approaches.
Why This Topic Matters
IMRT coding can involve multiple coordinated services across planning, device construction, and treatment delivery, so understanding the scope of each step helps support accurate reporting and documentation review. The article also helps readers understand when IMRT may be considered clinically appropriate and how related therapy planning concepts fit into the coding discussion.
