Are these services clinic or pharmacist specific? When it says "upon request," does the requesting party need to be documented in the pharmacist's documentation for each visit? For add-on code 99607, please clarify whether code 99607 may be reported in addition to either code 99605 or code 99606 for a 20-minute encounter.
