From a CPT coding perspective, code 27146, Osteotomy, iliac, acetabular or innominate bone, is intended to be reported only one time per bone, regardless of the number of sites in which osteotomy was performed. However, if osteotomies were performed on separate bones (eg, iliac and innominate), then it would be appropriate to report code 27146 two times, once for each bone.
