Ordering Requirement—Physician Signature The Centers for Medicare and Medicaid Services (CMS) provided clarification on the signature requirements for ordering diagnostic tests through Transmittal 94, released on August 29, 2008. This transmittal states that a physician's signature on a diagnostic test order is no longer a requirement in order for the test to be paid; however, the physician must document his or her intent that the test be performed on the medical report.
