Introductory guidelines with four parenthetical notes have also been added to provide instruction on the appropriate use of these three new codes. In the Cardiovascular/Other Therapeutic Services and Procedures subsection of the Medicine section, a parenthetical note has been added to reference the deletion of codes 92992 and 92993...
