What are the correct CPT codes to report the following procedures when performed at the same operative session: (1) a spinal accessory to suprascapular nerve transfer; (2) Oberlin nerve transfer (ulnar fascicle to musculocutaneous); (3) radial nerve to axillary nerve transfer; and (4) neurolysis of ulnar nerve, radial nerve, musculocutaneous nerve, axillary nerve, spinal accessory nerve, C5 nerve root, C6 nerve root, upper trunk nerve, and suprascapular nerve? All the procedures are being performed to treat brachial plexus palsy. Although code 64713 (neuroplasty) includes transfers and neurolysis, it does not reflect the complexity of the procedure; therefore, would it also be appropriate to report suturing codes for the additional nerves and branches with modifier 59 appended?
