Is the repair of an unavoidable iatrogenic injury separately reportable? During the primary excision of soft tissue skin sinus tract down to and including fascia and primary repair of the resulting unavoidable colotomy, it is not unusual to encounter scenarios in which the injury repair is performed by the same physician who performed the original procedure, while in other scenarios another physician is called in to perform the repair.
