A split thickness skin graft was taken from the right forearm. This was applied to two separate wounds,(1) measuring 6cm x 5cm and (2) 9cm x 6cm. Since there are two separate wounds, is it appropriate to report code 15100 twice? If yes, would you apply modifier 51 to the second 15100?
