Based on CPT coding guidelines for application codes 15271-15278, the skin substitute graft is "anchored using the individual's choice of fixation;" however, physicians sometimes apply skin substitute grafts without anchoring. For example, if a physician applies, but does not anchor, a skin substitute graft to a right anterior leg wound and then applies a non-adherent dressing over the skin graft to help hold it in place to the wound base, would it be appropriate to report an application code or is anchoring required to report codes 15271-15278?
