A stent was placed through a saphenous vein bypass graft into the first diagonal of the left anterior descending (LAD) artery. An additional angioplasty was performed through the saphenous vein bypass graft of the right coronary artery (RCA). The coronary therapeutics services and procedures guidelines state, PCI performed during the same session in additional coronary artery branches should be reported using the applicable additional add-on code(s). The second procedure did not involve a stent, and the narrative of both codes 92937 and 92938 indicates that any combination of stent, atherectomy, and angioplasty is reported with these codes. Is the scenario described reported as 92937 x2, or is it appropriate to report this scenario with codes 92937 and 92938?
