When doctors provide valuable medical care, and when patients seek medical care, there are many assumptions that take place.  The patient, provider, and medical biller all assume that because someone has a health insurance benefit card, that all medical care is covered.  The provider, patient and billing staff assumes that the claim is to pay the doctor and the doctor is required to submit the claim.  Another assumption is that when there are problems with claims, it is the doctor or the medical biller that must appeal the problem.  Some of this may be true...
