CMS and the Medicare Administrative Contractors (MACs) process claims from all 50 U.S. states, plus the U.S. territories. Sometimes, certain regions tend to log more errors than others. This year’s Medicare Fee-for-Service Supplemental Improper Payment Data document, which CMS issued on December 7, 2021, breaks down a list of which states were responsible for the most (and the fewest) improper payments.
