Lumbar Perforator Flap Breast Replacement
A patient with a history of bilateral mastectomy and implant reconstruction complicated by grade IV capsular contracture and bilateral seromas, was admitted for removal of the implants and bilateral capsulectomy. Breast reconstruction with lumbar artery perforator (LAP) flaps was performed with vein and artery grafts from the deep inferior epigastric artery. The deep inferior epigastric arteries and veins were harvested from the abdomen. The left L3 perforator and right L4 perforator were dissected and harvested. The LAP flaps were anastomosed to the deep inferior epigastric artery and vein grafts. The patient was turned supine, and the LAP flaps were placed on the left and right chest. On the right, the deep inferior epigastric artery was anastomosed to the right internal mammary artery, and the right deep inferior epigastric vein was anastomosed to the right internal mammary vein. On the left, the deep inferior epigastric artery was anastomosed to the left internal mammary artery, and the deep inferior epigastric vein was anastomosed to the left internal mammary vein. The flaps were without kinks or twists and had excellent flow. They were shaped and inset, in the native skin envelope of the breast.
