State Medical Record Association Questions and Answers

This column of questions submitted by state medical record associations is a new feature of Coding Clinic. The answers are being published as part of our overall effort to assist coders who need guidance with these issues. As with all material that appears in this publication, the answers have been approved by the Cooperating Parties (AHA, AMRA, HCFA, and NCHS).
