The Facets of Paravertebral Facet Joint Procedures (64470-64476, 64622-64627) (September 2004)

September 2004 pages 1-7 The Facets of Paravertebral Facet Joint Procedures (64470-64476, 64622-64627) More than a decade ago, it was identified that the tiny medial branch nerves supplying the paravertebral facet joints could cause severe and diffuse pain. Prior to CPT 2000, codes 64440-64443 were reported to describe lumbar paravertebral facet joint nerve injections performed in only the lumbar spine. Since facet injections are also performed on both the cervical and thoracic levels, paravertebral facet joint/facet joint nerve procedure codes 64470-64476 were added for CPT 2000 to more accurately reflect clinical practice. The two add-on codes in this series...

Article Overview

This premium article reviews reporting concepts for paravertebral facet joint procedures and related spine pain management services. It is aimed at coders, billers, and clinical documentation staff who need to understand how the article frames procedure terminology, spinal region differences, guidance reporting, and related diagnostic context. The discussion also references diagnosis coding examples, modifier use, and distinctions between facet injections and facet nerve destruction.

Why This Topic Matters

Facet joint and spine pain procedures often involve overlapping terminology, multiple spinal levels, different regions of the spine, and ancillary services such as imaging guidance. This article helps readers understand the scope of the reporting issues covered and the coding references discussed in the source.

Article Sections

  1. Indications for Treatment

Introduces the clinical context for facet-related pain and why these procedures may be considered. Also discusses the role of diagnostic evaluation in spine pain workups.

  2. The Goal of Treatment

Summarizes the general purpose of facet joint and facet nerve interventions in pain management. Focuses on the broad therapeutic and diagnostic intent of the procedures.

  3. Procedure Report Terminology: Facet Joint Nerve

Reviews terminology and documentation themes that appear in procedure reports. Highlights the need to distinguish among different spine-related approaches and anatomic targets.

  4. Paravertebral Facet Joint Injection(s): Single vs Multiple Levels

Addresses how the article discusses level-based facet injection reporting and spinal region distinctions. Includes broader discussion of unilateral and bilateral procedures and related documentation themes.

  5. Use of Modifiers

Covers modifier-related issues discussed in the article. Emphasizes general reporting considerations for bilateral and multiple-procedure scenarios.

  6. Injection(s) in Different Spinal Regions

Describes how the article distinguishes procedures performed in the cervical, thoracic, and lumbar regions. Focuses on region-specific documentation and reporting considerations.

  7. Single vs Multiple Needle Injection Technique

Explains how the source addresses technique variations in facet injections. Discusses how the article frames documentation when different needle approaches are used.

  8. Fluoroscopic Guidance and Needle Localization/Injection of Contrast Material

Reviews imaging-guidance and contrast-related topics associated with these procedures. The section also situates guidance reporting alongside the primary procedure discussion.

  9. Transforaminal Injection(s): Single vs Multiple Levels

Compares transforaminal injection concepts with other spine injection approaches discussed in the article. Covers the broader issue of multiple levels and region-based reporting themes.

  10. Facet Denervation vs Facet Joint Nerve Injection or Destruction Procedure(s)

Addresses the article’s distinction between facet injection concepts and denervation/destruction procedures. Discusses how the source frames the separate reporting category for nerve destruction.

  11. Facet Joint Injection Coding: Test Your Skills

Presents illustrative coding scenarios included in the article. The section groups the examples that apply the earlier reporting concepts.

  12. Facet Joint Nerve Anatomy

Provides an anatomic overview of the nerves associated with facet joints. Helps contextualize the procedure discussion in relation to spinal nerve branching.

  13. Glossary

Defines selected terminology used throughout the article. Supports reader understanding of the clinical and coding language referenced in the discussion.
