AHA Coding Clinic® for ICD-9 - 2004 Issue 4; VOLUME 3 NEW/REVISED PROCEDURE CODES
Percutaneous Angioplasty and Atherectomy and Percutaneous Stent Insertion
Effective October 1, 2004, a new code subcategory has been created to identify percutaneous angioplasty and atherectomy of precerebral and intracranial vessels as well as percutaneous stent insertion of other precerebral, carotid and intracranial arteries. Carotid artery occlusion is an obstruction or narrowing of the carotid arteries. These arteries provide blood from the heart to the brain. A frequent cause of carotid artery stenosis is atherosclerotic plaque that impedes blood flow into the brain, leading to stroke. The conventional treatment is carotid endarterectomy, a surgical procedure performed under general anesthesia. Recent advances in interventional radiology, have made possible endovascular...

Article Overview
This article covers a coding update effective October 1, 2004 that introduced new procedure codes for endovascular treatment of precerebral, carotid, and intracranial arteries. It is aimed at medical coders, coding educators, and reimbursement professionals who need to understand the scope of the new vascular procedure category and the broad circumstances addressed by the guidance.
Why This Topic Matters
The article matters because it organizes a newly created procedure code subcategory and identifies the vascular interventions and anatomical areas that fall within it. That makes it relevant for accurate code selection, coding education, and review of cerebrovascular interventional procedures.
Article Sections
-
Overview of the vascular procedure update
Introduces the effective date and the general purpose of the new procedure code subcategory. It also provides clinical context for carotid and related cerebrovascular disease and why endovascular treatment became relevant.
-
Endovascular therapy background
Describes the broad interventional approach used for treatment of narrowed vessels and the setting in which these procedures are performed. It discusses the general role of supportive devices used during the procedure.
-
New subcategory 00.6 Procedures on blood vessels
Summarizes the new subcategory and the surrounding exclusion framework for vascular procedures. It then outlines the newly introduced codes in the category and the associated anatomical groupings addressed by the update.
